-Observed boxes of product that were stored on the floor of freezer 1; freezer 2; and the cooler. Ensure all food products are stored at least 6 inches off the floor to prevent product damage and contamination.-In freezer 1; observed packaged ham that had fallen out of the original container onto the floor. Ensure product is properly contained so that it does not fall onto the floor. Operator voluntarily discarded product.-In freezer 2; observed packaged chicken that had fallen out of the original container onto the floor. Ensure product is properly contained so that product does not fall onto the floor. Operator voluntarily discarded product.-In freezer 1; observed broken open package of chicken livers sitting on top of cardboard box. Operator voluntarily discarded chicken livers. Upon further inspection; several packages of chicken livers in the original container had holes in the bottoms of the bags. Operator said the chicken livers are received that way and they are not re-packaged before shipping. All packages were frozen solid. If product bags are damaged; recommend putting damaged bag in another bag before shipping to prevent product from being contaminated or leaking onto other products.

Notes:-Restroom clean and stocked.-Freezer 1 at 20F. All product frozen solid. Operator was moving product and had the door open. Operator said the temperature of freezer 1 is normally at 10F.-Freezer 2 at -5F.-Cooler at 34F.-PCO is Terminex on a monthly routine. No pests observed.*Recommend keeping damaged product and product samples in separate labeled areas to prevent this product from getting mixed in with consumer products.

Operator staging cases of raw meats on the warehouse floor out of temperature control. Manager advised 3-4 hours on the floor on busy days is possible. Advised manager that the amount of time potentially hazardous foods are out of temperature control must be minimized. Recommend no longer than two hours. Operator to develop and institute a management plan to address this issue.

Thermometers are provided in the walk-in cooler and freezer however they are not NSF listed/certified. Install at least one NSF listed thermometer in the cooler and in the freezer. You may continue to use the large easy to read thermometers and utilize the NSF units as a standard that the large units must meet.

The meat typing box is not cleanable (raw wood) and has staining from spills during use. Surfaces must be designed so that they are non-absorbent and easily cleanable. Either replace the typing box with a cleanable design or cover; paint or seal the raw wood so that it is easily cleaned.
- General Comments that relate to this Inspection
OBSERVATIONS; REQUIREMENTS AND RECOMMENDATIONS:- The Certified Food Protection Manager certificate or a copy must be posted within the facility.- The Pest Control Operator is treating once/monthly (Terminex). This frequency appears to be adequate upon records review as there were little to no activity noted within the facility. Advised owner to follow recommendation noted provided by the PCO on each treatment report as a cooperative effort by the facility and PCO is most effective.- Provide a covered trash can in the women's restroom for sanitary napkin disposal purposes.- Store all chemicals away from foods (e.g. bleach bottle on rack between loading doors).- All cooler temperatures were at or near that required by regulation (walk-in 32F; freezer 3F).A re-inspection will occur on or after 12-7-14 to verify compliance.
