Violation descriptions and comments

The infant room was not provided with a thermometer for measuring the temperature at floor level. The floor below the hand sink in the toddler 2 area was carpeted. The wall coving by the door in the 4-5 year room was not secured. The door laminate between the toddler1 and toddler 2 rooms was loose.
The toddler 1 hand sink did not have hot water at a temperature of 90�°-120�°F.
The toddler 1 area did not have a stool provided for the hand sink.
Sleeping mats in the toddler 1 and in the 4-5 year old classrooms were not labeled and staff stated they were sanitized weekly.
Staff stated in the infant room, in the toddler 2 room, and in the 4-5 year old room that soft toys and pine cones were sanitized 1-2 times a month.
The staff in the infant room stated that they were not separating ill children from the room.
An employee was observed washing a cup in the diaper hand sink. In the 3 year old room, a plastic chair and foam blocks were soiled with grime, and the fabric stroller was not washable. In the 4-5 year old room the pack n play was not cleanable. In the infant room, exersaucer seats were dirty, the stuff animal bunny was electronic and not machine washable, and the adult cloth chair was cloth covered. In the toddler 1 room the soft doll was soiled with grime.
Diaper changing disinfectant in multiple classrooms had concentrations of less than 200ppm chlorine. Toy and tabletops sanitizer in multiple classrooms had concentrations of less than 50 ppm or more than 200ppm chlorine.
Food worker did not know the proper cooking temperature of chicken. Milk in the infant room reach-in refrigerator and in the kitchen refrigerator was 52�°F. Hot dogs in the kitchen refrigerator were 44�°-61�°F.
The kitchen standing freezer shelves were broken.
The strap on the changing table in the toddler 2 room was cloth.
Bubbles were stored in reach of children in the school age classroom.

The hot water at the toddler1 hand sink was less than 90�°F. A hand sink in the main part of the child care center was was taped off.
The hand sink in the toddler 1 area did not have a stool and was out of reach for toddlers.
Diaper changing bleach in the infant room and in the toddler 1 room had a concentration of less than 200ppm chlorine.
Milk was 46�°F and soy milk was 47�°F in the infant room refrigerator.
