Violation descriptions and comments

Tomatoes were prepared/cut next to raw meat in the kitchen. Dented cans were stored next to wholesome food on the shelves outside of the walk-in cooler.
An open employee beverage and twist top employee beverages were stored above and next to equipment by the stove hand sink.
Tamales in deep (4 layers) containers in the kitchen walk-in cooler were 77-88�°F after cooling for more than two hours.
Cut tomatoes on the kitchen prep table were 71�°F. Meat in the restaurant reach-in cooler was 46�°F. Sausage, steaks and ground beef in the meat display case were 47-50�°F. Salsa with cut tomatoes in the customer area was 48�°F. Eggs in the tortilla area reach-in cooler were 56�°F. A pastry with cream in the display case was 45�°F.
Hot water was not available at the facility.
Numerous flies were observed in the meat department and kitchen.
Chemicals were stored by knives in the meat cutting area and on top of the meat cold case above food. Lighter fluid was stored next to a box of raisins in the dry storage area.
Scented bleach was used in sanitizer solution in the restaurant.

An employee was observed carrying a hand full of raw beef and then opened the walk-in cooler door. Another employee with clean gloves then opened the door without knowing it and contaminated their gloves.

An employee was observed cracking shell eggs and handled the walk-in cooler door handle without first washing their hands.
An employee was observed wiping their gloved hands on their apron after handling raw meat and then removed their gloves.
Employee beverages were stored on a shelf in the walk-in cooler and on a prep table next to customer food.
Fish thawing in the three-compartment sink was 57�°F. Eggs in the bakery reach-in cooler were 56�°F. Arroz con leche in the bakery cold case was 49�°F. Chorizo in the cold case by the meat area was 52-60�°F.

Raw vegetables were being prepared next to raw meat without a barrier in the kitchen.
Meat empanadas in a fryer basket were 127�°F.
Shrimp thawing in the three-compartment sink were 49-70�°F.
A live cockroach was observed above the three-compartment sink.

Raw shrimp was stored above cooked beans, shredded cooked beef and cooked tamales in the main kitchen walk-in cooler. Raw shrimp was stored next to avocados and raw beef was stored above cooked potatoes in the restaurant reach-in cooler.
An employee was observed handling/cutting cantaloupe with bare hands.
Cooked green salsa next to the stove in the kitchen was 74�°F. Chicharrones with meat in the meat hot box were 95-100�°F. Carnitas in the restaurant steam table was 122�°F.
Milk and flan in the bakery reach-in cooler were 47�°F and 46�°F respectively. Flan in the bakery case was 47�°F and was dated 12-9-13.
Several cockroaches of variable sizes were observed behind the ovens/proofers in the bakery area.

A live cockroach was observed behind the ovens/proofers in the bakery area. Dead cockroaches were also observed.
