Shelves in the service line reach-in refrigerator were chipped. The interior of the ice machine was soiled. A dual check backflow prevention device was not installed on the water line for the coffee machine. The floor throughout the entire establishment was damaged and in poor repair. The wall at the three compartment sink was damaged. The lights in the dry storage area were not shielded. Several unused pieces of equipment were stored in the dry storage area. More than 15 seats were present in the facility while only customer restroom was available.
