Employee beverages with screw caps were present in the production area and in the walk-in cooler above food items.
In the sandwich cold-top: cheddar was 50�°F, provolone was 50�°F.
Hot and cold water were not provided at the production area hand sink.
In-use sanitizer in the 3-compartment sink contained greater than 200 ppm chlorine.
A domestic blender was in use in the sandwich prep-area.
The restroom cabinet doors were in poor repair.
The vent hood panels were soiled.
The scoop handle for the bulk sugar on the production line was stored submerged in the product.
An ASSE 1022 dual check valve was not provided on the water line supplying the iced tea machine in the sandwich prep-area.
