- Food Safety Certificate is not available. Facility is repackaging bulk food items. Obtain the 5 year Food Safety Certificate within 60 days. Once obtained, fax or e-mail a copy of the certificate to Kadeanne Linhares klinhares@sjcehd.com Informational sheet provided.
- Comment:
- All food employees shall have adequate knowledge of and be trained in food safety as it relates to their assigned duties. (113947) Food facilities that prepare, handle or serve non-prepackaged potentially hazardous food, shall have an employee who has passed an approved food safety certification examination. (113947-113947.1) Any food handler hired after June 1, 2011 shall obtain a Food Handler Card within 30 days (113948).

- Details:
- Re-packaged food (nuts, dried fruit, beans, etc). Lack proper labeling. Labels shall include the following: 1) The common name of the food. 2) If two or more ingredients are included, list all ingredients in descending order by weight (include declaration of artificial color or flavor and chemical preservatives). 3) An accurate declaration of the quantity of contents. 4) The name and place of business of the manufacturer, packer, or distributor.

- Any food is misbranded if its labeling is false or misleading, if it is offered for sale under the name of another food, or if it is an imitation of another food for which a definition and standard of identity has been established by regulation.
