Fish from an unverified source was in the walk-in cooler.
Bottles of liquor at the bar were adulterated with insects.
Undercooked meats are served at the facility and the facility does not have a consumer advisory.
An employee was observed handling a dirty dish and then put on gloves without washing their hands.
Twist top beverages were in the domestic refrigerator next to customer food and on a prep table.
An employee stated that a meat grinder part is only cleaned every couple of days and was stored the the walk-in cooler soiled with meat in the food contact part.
A hotel pan of beans on the stove were at 92°f. French fries stored by the stove were at 67°f.
A pot of soup in the walk-in refrigerator was at 46°F. Sour cream, ranch dressing, and thousand island dressing were at 47°F in walk-in refrigerator. Cut tomatoes were at 52°F in the walk-in refrigerator. A block of cheddar was at 45°F in standing domestic refrigerator in the kitchen.
Quarterly bacteria tests for the well were not available during the inspection.
The sprayer above the dish washing area was not secured above the flood rim.
Rodent droppings were observed in the back area by the surveillance camera on the top shelf.
Fly strips were stored over a food preparation table in the dish washing area.
Hand sanitizer was stored on a food preparation table in the kitchen. Scrubbing Bubbles chemical was stored over styrofoam cups and next to napkins. Butane was stored over the preparation table in the dish washing area.
Chlorine sanitizer bucket contained over 200ppm residual chlorine.
An uncovered container of dried lavender was stored on the top shelf of the back dry storage area. Loaves of bread and chicken breasts were stored on the floor of the walk-in freezer. Buckets of cut potatoes were stored uncovered on the walk-in refrigerator floor.
A non-commercial use griddle was stored on a shelf in the ware-washing room. A non-commercial roaster was stored in the back room by the ice machine. A non-commercial crockpot was stored in the back storage area by surveillance television. A non-commercial food processor was stored on a preparation table in ware-washing room. Potentially hazardous food was stored in a non-commercial refrigerator in the kitchen. Bread was stored in non-food grade bags in the walk-in freezer. Shelves in the walk-in freezer were rusty.
The utensil storage drawer under the preparation table in the ware-washing room was rusty. A plastic bag was covering the electrical box of the dish washing machine. The dry storage shelves in the dry storage area were rusty. Particle board lined the walk-in freezer shelves. The fan shield in the meat walk-in refrigerator was broken.
The three compartment sink was not sealed to the wall.
No chlorine test strips were available for testing sanitizer.
Clean knives stored in the knife holder in the ware washing area were soiled with food debris. The inside of the microwave on the preparation table in the kitchen was soiled with food debris. The inside of the microwave next to the cook line was soiled with food debris. Metal baking sheets were stored soiled on a shelf outside of meat walk-in refrigerator. The table-mounted can opener blade was soiled with food debris. The bar soda gun dispenser was soiled with mold.
The shelves in the walk-in meat refrigerator were soiled with food debris. The walk-in freezer floor was soiled with food debris. The floor under the dry storage shelves were soiled with food and dirt. The floor in the walk-in meat refrigerator was soiled with food and dirt. The knife holder on the wall by the preparation table in the ware-washing room was soiled with food debris . The fan guard in the walk-in freezer was soiled with dust. The vent above the dish washing machine was soiled with dust. The kitchen light shields were soiled with dust. The fan in the walk-in meat refrigerator was soiled with dust.
Mayonnaise buckets were being reused for sugar and flour storage.
A pipe was leaking into a bucket outside the walk-in meat cooler. The three compartment sink faucet leaked in the off position. The caulking on the three compartment sink was moldy
The walls behind the dish area were soiled with food debris and dirt. Tiles around the ice machine were cracked. The wall by the potato storage area was soiled with grime and goo. The wall behind the ice machine was soiled with dirt. Spider webs were observed in the corner of the walls outside the meat walk-in refrigerator. The floor under the dish area was soiled with food debris and dirt. The floor under the three compartment sink in the kitchen was soiled with food and dirt. The tiles around the kitchen hand sink were cracked.
Lights over the three compartment sink were burnt out. Two lights were burnt out and unshielded in the dish washing area.
Personal items (keys, paycheck) were found stored on the preparation table in the ware washing room.
The door to the outside in the dish washing area was open and not close-able. An unused hot holding unit was present in the back storage area next to the ice machine.

A description of the animal-derived food that are undercooked and the statement "consuming (specific food items) may increase your risk of foodborne illness, especially if you have certain medical conditions" was not part of the consumer advisory posted by the front door.
An employee was observed handling dirty dishes and then clean dishes without first washing their hands.
An unapproved employee beverage was stored on the kitchen prep table.
Cooked fries were labeled with a time of 1pm and were 71°F. They were checked at 225pm which was past the time labeled on the fries.
Quarterly bacteria tests for the well were not available during the inspection.
The sprayer on the warewashing machine was not completely secured to the hook which would allow it to hang below the flood rim.
Rodent droppings were observed in the back area by the surveillance camera. Numerous flies were observed in the kitchen.
