  • Beverage tubing is in contact with stored ice in the bar area.
    To prevent cross-contamination, beverage tubing and cold-plate beverage cooling devices may not be installed in contact with stored ice. Install ice guard on beverage tubing to protect ice from possible contamination.
  • Equipment components are not intact, tight or properly adjusted. [Door gaskets on the two-door prep cooler are damaged/missing.]
    Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Non-food contact surfaces of equipment are dirty (ie. hood system above cookline, cookline and prep equipment, and fan guards in the walk-in-cooler.
    Nonfood-contact surfaces of equipment shall be kept clean. Thoroughly clean the exterior and interior of cookline equipment/coolers, hood system, and fan guards to prevent possible contamination and pests.
  • Floors, walls, and/or ceilings were not smooth and easily cleanable. [Observed serveral cracked/chipped floor tiles in the kitchen]
    Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable. Repair/replace floor tiles in the kitchen to ensure that the flooring is smooth and easily cleanable.
  • Observed no service sink or curbed cleaning facility. [Service sink is currently blocked by the two-door freezer]. Per discussion with PIC, mop water is emptied in the three compartment sink.
    A service sink or curbed cleaning facility shall be provided as specified under paragraph (I) of rule 3717-1-05.1 of this code. Ensure that utility sink is accessible and discontinue use of three-compartment sink for disposing of mop water.
  • Observed unnecessary or nonfunctional items and /or litter on the premises. Non-functional equipment is being stored in the kitchen area.
    The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. [Remove non functional equipment/coolers from the kitchen area.]

  • Critical: Observed food that was not properly protected from contamination by separation, packaging, and segregation.
    To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule. [Raw foods were stored above ready to eat foods.]
  • Critical: Refrigerated, ready-to-eat, TCS foods held refrigerated for more than 24 hours were not properly date marked.
    To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. The day the food is prepared or opened is day one. [Food items must be marked with the date made, the date frozen, and the date thawed.]
  • Beverage tubing is in contact with stored ice.
    To prevent cross-contamination, beverage tubing and cold-plate beverage cooling devices may not be installed in contact with stored ice.

    Nonfood-contact surfaces of equipment shall be kept clean. [Thoroughly clean shelves, the exterior of all equipment, and any other non-food contact surfaces.]

    A service sink or curbed cleaning facility shall be provided as specified under paragraph (I) of rule 3717-1-05.1 of this code. [Move the reach in freezer to make the service sink accessible.]
  • The physical facilities are not being maintained in good repair.

  • Observed a build-up of dirt and debris.
    The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed. [Thoroughly clean the floor under and around all equipment.]
