- Critical: Observed food employee touching ready-to-eat food with bare hands. Pizza and wings were touched with bare hands when boxing.
To prevent contamination, except when washing raw fruits and vegetables, food employees may not contact exposed, ready-to-eat food with their bare hands and shall use suitable utensils. As discussed, all ready-to-eat foods must be handled with utensils or gloved hands.
- Observed plumbing fixtures that were not being kept clean and maintained. The hand sink and it's components are dirty to sight and touch.
Plumbing fixtures such as handwashing sinks, toilets, and urinals shall be cleaned as often as necessary to keep them clean and maintained.

The premises shall be free of Items that are unnecessary to the operation or maintenance of the FSO or RFE such as equipment that is nonfunctional or no longer used and litter. Remove the non-functioning reach in prep cooler from the premises.

- Food is not protected from environmental sources of contamination during preparation. Observed a bag of onions and box of oranges being stored directly on the floor.
During preparation, unpackaged food shall be protected from environmental sources of contamination. Produce was moved off of the floor during the inspection. Per PIC, a new cooler will be delivered within the month that will provide for more storage of produce items.

To prevent foodborne illness, refrigerated, ready-to-eat, TCS food held at a temperature of 41°F or less for more than 24 hours shall be clearly marked at the time of preparation or the time the original container is opened to indicate the date or day, that is a maximum of seven days, by which the food shall be consumed, sold, or discarded. - Critical: The handwashing sink was not easily accessible. Handwashing sink had clean dishes drying in the sink.
To ensure frequent and proper handwashing, a handwashing sink shall be maintained so that it is accessible at all times for employee use. Do not use the hand sink for any purpose other than washing hands. Do not place items in handsink that prevent the sink from being easily accessible when needed.
- Critical: The plumbing system was not properly repaired. The bathroom hand sink is not operational at time of inspection
To prevent health hazards, a plumbing system shall be repaired according to the Ohio building code. Operator stated that a broken pipe in the wall was in the process of being repaired, and that the toilet has been hooked back up but that the hand sink has not. Repair the handsink to eliminate the risk of cross contamination from washing hands in handsink in prep areas.
