- Observed accumulation of soil residue on nonfood-contact surfaces. - Observed a build-up on baffle of ice machine.
Nonfood-contact surfaces of equipment shall be cleaned at a frequency necessary to preclude accumulation of soil residues. - Clean and sanitize baffle to remove build-up and prevent contamination of ice.
- Observed light bulbs or heat lamp that were not properly shielded or coated where required. - Observed exposed light bulbs on ceiling of walk-in freezer.
Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.
- Observed a build-up of dirt and debris. - Observed a build-up of debris on floor under ice machine and on floor of walk-in freezer.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

- The light intensity in the walk-in refrigerator, dry food storage area, or other area during cleaning was less than ten foot candles. -Observed insufficient light in the walk-in freezer.
The light intensity shall be at least ten foot candles (one hundred eight lux) at a distance of thirty inches (seventy-five centimeters) above the floor, in walk-in refrigeration units and dry food storage areas and in other areas and rooms during periods of cleaning. -Replace burned out and faded bulbs to provide light inside the walk-in freezer.

- Observed food employees wearing jewelry on arms or hands during food preparation.
To prevent contamination, except as provided by (G)(2) and (G)(3) of this rule, food employees may not wear jewelry on their arms or hands.
- Cooking equipment or pan surfaces are dirty. The bread toaster has a build up of crumbs on the inside, and a build up of dust on the outside.
To prevent contamination, the food-contact surfaces of cooking equipment and pans shall be kept clean. Thoroughly clean the bread toaster to prevent cross contamination.
