- Equipment and/or components are not maintained in good working order. Repair/replace open cooler to prevent product from freezing and thawing. Owner stated that a part is on order.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
- Critical: The plumbing system does not comply with the Ohio building code. Repair/ replace grease trap to no longer leak. Owner stated the grease trap will be replaced by the end of the week.
To prevent health hazards, a plumbing system shall be designed, constructed, and installed according to the Ohio building code.
- Observed inadequate number of toilets and urinals. Repair/ replace toilets in both restrooms to fill properly after flushing. Women's fills slowly. The owner had to remove the back of the men's toilet and handle the components inside to have the water begin to trickle into the toilet.
Toilets and urinals shall be provided according to the Ohio building code.
- Floors, walls, and/or ceilings were not smooth and easily cleanable. Repair the following to be smooth and easily cleanable: - Floor where damaged (such as near prep sink, and near side door) - Wall and shelf above prep line -Basecoving where missin or damaged -Doorway/ basecove area at walk in cooler door
Floors, floor coverings, walls, wall coverings, and ceilings shall be designed, constructed, and installed so they are smooth and easily cleanable.

- Observed the use of an unapproved sewage drainage system. Repair wasteline in walk in freezer to prevent ice buildup and possible contamination of food.
FSO or RFE drainage systems, including grease traps, that convey sewage shall be designed and installed as specified under paragraph (B)(1) of rule 3717-1-05.1 of this code.
- Critical: Observed working containers of poisonous or toxic chemicals that were not properly labeled with the name of the material. Found spray bottle without a label.
Working containers used for poisonous or toxic materials such as cleaners and sanitizers taken from bulk supplies shall be clearly and individually identified with the common name of the material.
- Critical: Observed the use of a chemical sanitizer that is not approved for use on food contact surfaces. Unmarked bottle of bleach/water solution had a concetration of >200 ppm.
Chemical sanitizers and other chemical antimicrobials applied to food-contact surfaces shall meet the requirements specified in 40 CFR 180.940.
