Non-food contact surfaces of equipment are dirty. Observed a build-up of food debris inside unplugged coolers, being used for storage. Nonfood-contact surfaces of equipment shall be kept clean. Clean inside of coolers not in use, to provide a sanitary surface and deter pests.
The plumbing system was not properly maintained. Faucet (motion activated) in hand sink closest to entrance of women's restroom is not working. Urinal in men's restroom having issues with flushing. A plumbing system shall be properly maintained. Repair faucet and men's urinal.
Observed light bulbs or heat lamp that were not properly shielded or coated where required. Provide a shield for temporary lighting (light bulbs) in walk-in meat cooler, or repair permanent lighting fixtures. Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.

Critical: Mashed potatoes in the "Henny Penny" case were not being held at the proper temperature. TCS food shall be held at 135°F or above to prevent foodborne illness. Note: Ensure there are no "voids" in the hot case wells so hot air does not escape, and heats foods from below. Corrected: A new batch of mashed potatoes were re-made, and the voids were filled.
