Light bulbs shall be shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, or linens, or unwrapped single-service or single-use articles and does not apply in areas used only for storing food in unopened packages. An infrared or other heat lamp shall be protected against breakage by a shield surrounding and extending beyond the bulb so that only the face of the bulb is exposed.

The physical facilities shall be maintained in good repair.
- Observed a build-up of dirt and debris. Dirty fanguards in the walk in cooler (basement)
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

- Critical: TCS foods were not being held at the proper temperature. Food items in show cases were at 46 to 50 F. Food removed and put into proper refrigeration.
To prevent the growth of pathogens, except during preparation, cooking, or cooling, or when time is used as a public health control, TCS food shall be held at 135°F or above or at 41°F or less. Temperatures of food product should be taken not equipment to insure proper cold holding.
- Equipment and/or components are not maintained in good working order. Show cases food temperatures at 46-50 F. Cold holding equipment shall be maintained to keep food product at 41 F or below.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
- Equipment food-contact surfaces or utensils are dirty. Dairy show case has a build up of food debris.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch.

- Observed improper storage of food items. Observed several food items (ex: pudding cups, flour, cans) being stored on floor of sales isle.
Food shall be protected from contamination by storing the food in a clean, dry location, where it is not exposed to splash, dust, or other contamination, and at least six inches above the floor. Food items removed from floor.
- Foods in frozen storage exhibit signs of thawing or refreezing. Observed frozen foods stacked above the load line. Bags of Super Pretzels were beginning to thaw.
Stored frozen foods shall be maintained frozen.
- Critical: Equipment food-contact surfaces or utensils are dirty. Observed a build-up of liquid and debris on liner of egg and dairy coolers. Observed a build-up of dirt and debris on bottom of deli/hotdog cooler, under metal rack.
To prevent contamination, equipment food-contact surfaces and utensils shall be clean to sight and touch. Replace liners of egg and dairy coolers, and clean interior bottom of deli/hotdog cooler to provide a sanitary surface and prevent contamination of food.

Stored frozen foods shall be maintained frozen. Discontinue stacking foods above the load line to allow for proper air flow and to allow for foods to be kept frozen.
- Critical: (CORRECTED AT TIME OF INSPECTION) Unsafe food was not discarded or properly reconditioned. Observed numerous dented cans offered for sale on the sales floor. Also observed two out of date (Exp May 2014) baby formula cans behind the customer service counter.
A food that is unsafe, adulterated, or not honestly presented as specified under 3717-1-03 of this code shall be discarded or reconditioned according to an approved procedure. Rotate product often, and remove dented cans from sales floor to prevent the sale of potentially hazardous foods. Correct By: 19-Jun-2014
- Non-food contact surfaces of equipment are dirty. The egg cooler had broken eggs on lining of cooler.
Nonfood-contact surfaces of equipment shall be kept clean. Clean the egg cooler lining to prevent bacterial growth and cross contamination.
