- Equipment components are not intact, tight or properly adjusted. Magnetic strip/gasket of 2-door reach-in cooler does not properly seal right door to cooler.
Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications. Repair cooler so that doors properly seal to cooler, allowing proper insulation and preventing spoilage of food.
- The physical facilities are not being maintained in good repair. Insulation covering piping in dry storage rooms is damaged, exposing fiberglass insulation and pipes.
The physical facilities shall be maintained in good repair. Replace insulation to prevent possible contamination of food by fiberglass.
- Observed a build-up of dirt and debris. Observed a stain on floor located near milk coolers.
The physical facilities shall be cleaned as often as necessary to keep them clean, and cleaning shall be done during periods when the least amount of food is exposed.

The physical facilities shall be maintained in good repair. Replace pipe insulation to eliminate the potential for foods and/or surfaces from becoming contaminated with fiberglass.
- The ventilation system was not being properly cleaned. (ie. ceiling ventilation grate is covered with dust/dirt in the employee restroom).
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge. Clean ceiling ventilation grate in the employee restroom.
- Critical: Observed improper display or storage of poisonous or toxic materials offered for retail sale (ie. container of ant and roach powder/boric acid is sitting on the floor, in southeast corner, of dry storage room).
Poisonous or toxic materials shall be stored so they cannot contaminate food, equipment, utensils, linens, single-service articles, or single use articles by separating the poisonous or toxic materials, and locating the poisonous or toxic materials in an area that is not above food, equipment, utensils, linens, single-service articles, or single-use articles. Remove container of ant and raoch powder-boric acid from floor of the dry storage room. Any pest control measures in the school should be addressed by your contracted and licensed pest control operator. Corrected at time of inspection - Removed. Correct By: 19-Mar-2014
