Observed food packages that are not properly labeled. - Observed packages of pepperoni and spinach pies being stored in freezer without labeling. To properly inform consumers, packaged food shall be labeled as specified in law. - Per conversation with owner, pies are received from former store owner. He was not sure as to where former owner was preparing pies (home vs. licensed facility). Discussed requirement of preparing food in a licensed facility. Owner will no longer accept pies if they are being home-produced. Reviewed requirements for labeling of pies. A list of labeling requirements will be mailed to owner.
Critical: (CORRECTED DURING INSPECTION) Observed food that was not properly protected from contamination by separation, packaging, and segregation. - Observed cartons of raw eggs stored over milk and other beverages in reach-in cooler. To prevent contamination, food shall be protected from cross contamination by separating raw animal foods during storage, preparation, holding, and display from cooked ready-to-eat food as described in this rule.

Non-food contact surfaces of equipment are dirty. - Observed a build-up on shelves of reach-in beverage cooler. - Observed a build-up of dust on condenser fans in walk-in cooler. Nonfood-contact surfaces of equipment shall be kept clean. - Clean shelves of beverage reach-in and condenser fans of walk-in to prevent contamination of food.

Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code. -Repair ice freezer to eliminate leaks. Discard bags of ice that have been damaged or contaminated.
