- Equipment and/or components are not maintained in good working order. Repair/ replace beer reach in cooler to remove buildup in the bottom of cooler.
Equipment shall be maintained in a state of repair and condition that meets the requirements specified under rules 3717-1-04 and 3717-1-04.1 of this code.
- The ventilation system was not being properly cleaned or was creating a public health hazard. Clean cookline hood to remove buildup.
Intake and exhaust air ducts shall be cleaned and filters changed so they are not a source of contamination. If vented to the outside, ventilation systems may not create a public health hazard or nuisance or unlawful discharge.

- Critical: Observed food employee touching ready-to-eat food with bare hands. Employee handled lettuce for salad without gloves. Hands washed and tongs used.
To prevent contamination, except when washing raw fruits and vegetables, food employees may not contact exposed, ready-to-eat food with their bare hands and shall use suitable utensils.
- Observed employees lockers located where food, equipment, or utensils may be contaminated. Found personal items such as purse, keys, etc. stored on prep table while food was being prepared. Items moved and areas cleaned and sanitized.
Lockers or other suitable facilities shall be located in a designated room or area where contamination of food, equipment, linens, and single-service and single-use articles cannot occur.

- Critical: Observed the air gap between a plumbing fixture and a piece of equipment that was not sufficient to prevent back siphonage of contaminated water. Install air gap on wasteline of prep sink.
An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (25 millimeters). Correct By: 09-Dec-2014
- Observed an outer opening, window, or door without proper protection against the entry of insects and rodents. Repair/ replace backdoor to seal properly.
To prevent contamination, outer openings of an FSO or RFE shall be protected against the entry of insects and rodents as specified in this rule.
