  • Observed kitchen rear exit door handle missing.
  • Observed reach-in cooler gasket torn/in disrepair, cook line unit's. Repeat Violation.
  • Observed wall in disrepair, dish area has some tiles missing and area under spray nozzle section.
  • Critical - Outer openings not protected with self-closing doors, rear exit door. Repeat Violation.
  • Critical - Portable fire extinguisher not properly mounted at least 4 inches off the floor and the top no higher than 5 feet off the floor, 2 fire extinguishers on the floor near managers desk in kitchen. For reporting purposes only.

  • Critical - Bathroom not enclosed with tight-fitting, self-closing doors, employee restroom.
  • Carbon dioxide/helium tanks not adequately secured, at the bar area.
  • Ceiling not smooth and easily cleanable, accousticle "offfice" type ceiling tiles installed in the kitchen.
  • Critical - Handwashing cleanser lacking at handwashing lavatory, bar area handwash sink.
  • Critical - No handwashing sign provided at a handsink used by food employees, bar area handwash sink.
  • Critical - No proof of required employee training provided. All public food service establishments must provide the division with proof of employee training upon request, including, but not limited to, at the time of any division inspection of the establishment. Observed photo copied certificates provided as proof of employee training- Division can only accept original certificates, No photo copies. This violatiin must be in comoliance by next unannounced inspection.
  • Critical - Observed a soup urn for employees to ladle soup from on a cart in the dining room. Soup urn must be moved either moved to area behind the bar or kitchen.
  • Critical - Observed dented/rusted cans, dented can of sauerkraut on the dry food storage shelf.
  • Critical - Observed food stored on floor, case of raw shell eggs and cases of produce in the walk in cooler. Corrected On Site.
  • Observed reach-in cooler gasket torn/in disrepair, cookline unit.
  • Critical - Outer openings not protected with self-closing doors, rear exit.
