A lot of them either know nothing about it or hide it outright from the parents. I have known so many people who adopted and were told they just needed to set strong boundaries and give the child lots of love and the child will come around. Later, after the adoption has gone though, they get diagnosed with RAD. There is help for RAD and attachment issues...ignoring them is not part of the treatment plan. And strong boundaries and lots of love doesn't cut it either.
One myth that is still strong out in the adoption community is that a child cant have attachment disorders (not necessarily RAD, but insecure attachment) if they were placed with their adoptive family before 6 months or before 18 months...I have even seen the age 3. Children can be born with attachment problems, let alone have them develop in their early life.
