NT-BNP (PRO BRAIN NATRIURETIC PEPTIDE) - BNP levels go up when the heart cannot pump the way it should. A result greater than 100 pg/mL is abnormal. The higher the number, the more likely heart failure is present and the more severe it is. Sometimes other conditions can cause high BNP levels.- Mount Sinai

I'm not used to seeing Test results that are so far out of the standard range. These results are for my father (age 86). Should I be alarmed?
I found a paper that said High BNP levels (4000-20,000 pg/mL) is determined more by renal dysfunction than by the severity of heart failure.
Serious enough to either question the cardiologist or get another one. Asking for very specific info on medical issues on the internet is not a reliable source.
Sounds as if your father is seriously ill whether heart or renal or even both.

Had this same situation with my MIL, her numbers were around 54,000!
She has CKD and CHF, they were able to bring her numbers back down with medication.
Took awhile but they did come down, never to "normal" though
The main value of getting BNP levels is as an estimate of prognosis. Occassionally it helps decide if mild CHF is present as opposed to some other cause of SOB/edema/wheezing. It can also confuse the issue because CHF isn't the only cause of elevations.
Here's a concise summary about it....An important quote from down in the Caveats secion--
"It appears that BNP at particular cut-points may be useful for predicting or anticipating clinical events but does not replace clinical judgement." (emboldening mine)
I don't think getting BNP levels have ever added a day to any pt's life.
