I am looking at the USPS Change of Address form online, and it seems to imply that I'll need to pay $1 for every variation of my name. Is this correct?
Select Individual and Repeat the Change of Address Process for each name if:
• You receive mail by more than one name (maiden name, married name or nickname)
If I have a first, middle & last name and suffix, but my mail is sent to several different combinations of those, then is it sufficient to provide only one form for my full name?
These are all the variations of my name I've found so far on mail I receive:

The $1 charge is the PO's way of verifying your identity, not a charge per name for the forwarding service.
Are you the only person with LAST name at the old address ?? IF yes, then just enter last name as if you were a family of different FIRST names wanting all mail for the LAST family forwarded. Then they don't care what first, middle, etc are, all mail for LAST gets forwarded.
The instructions you are looking at are for completely different last names. Sue Smith got married recently and is now Sue Jones, etc
reed is correct, ythey are really looking for what amoutns to different names like
John and Jane Doe gets mail under the following
John and/or Jane Doe (primary names that don;t need changes for every variation)
Jane Smith (maiden name)
John Frankiemachino (old withness protection name)
Bigg D and Lucy Whappo (names used on their porno site)
Excellsior Investments and Financial Services LLC (Caymen island company they get mail for)

It's typically a full last name, then you have the option of just change an individual with that last name, or the entire family w/ the same last name. But you can put whatever you want - as long as verifiable.
This gives an option if a child or an individual w/ the same last name is moving out, they can just forward their mail vs. everyone else's in the family that may not be moving.
You can certainly add another name completely - but if you're getting mail with just a last initial. You may be better off updating that with the sender.
And this is the other thing - when you move, you should update your address with those you do personal business with anyways (credit card, banks, businesses, etc.). Many organizations will request your new address via the USPS. But why wait when you have the options to update it as well?
When I moved, I didn't get too may "forwarded" mails since I updated most of mine directly. And most that were forwarded were unsolicited mail or those that were sent to the old one for verification purposes.
And yes - the $1 is to verify your identity. The forwarding service is free for a period of time (6 month or a year?). If you want it for longer, it's like $20. It's no longer free indefinitely like it used to be.
Another aspect to consider. Providing the USPS with the specific variations on your name that you DO want delivered helps them weed out the unsolicited junk mail you DON'T want filling your mailbox.

I was specifically wondering whether they would return mail even if I did mail forwarding with the first name + last name initial only or if this was even a possibility. But of course I could inform the sender/s.

My only concern would be that without the last name on the mail, if a number is off or something or blurred in some way, they would not be able to see the mail was for John Smith, but John S. In smaller towns, they will usually get the mail to you if there is enough info. I checked and it was saying that all they care about is the address for delivery, but again, if blurred or off a number, the name might get the mail to the right place.
