Possible water damage (how much, roof, drain, cleaning)

Looks like someone skimped and used a minimal drip edge instead of the proper flashing. Most any decent roofer can fix that. It needs to be done quickly, before you get structural damage.
OK, they are flat tiles( flat as opposed to s shaped).
Here are two options:
1) You could have a broken, slipped or displaced tile(s) anywhere along or directly near the valley.
2) IF none of the items in option one are present then you could have what is known as mud damming.
Silt from dust collects at the bottom of the tiles at the lower lugs( they are like raised ridges that support the bottom edge of the tile). These silt deposits form raised "dams" or diverters that improperly direct water up and over the lip /edge of the valley at the the roof then drain improperly as per your photo.
3) Damming or improper valley drainage can also occur due to valley apex clogging or valley area clogging due to tree leaves or needles and/or dirt.
Options 2 & 3 require dis-assembly of the valley, cleaning, inspection(possible repair) and proper reassembly.
