In my experience, the test phishing emails can be identified from inspection of the email headers. There's usually a string to track replying or forwarding and another to whitelist the test email through anti-spam filters. But the tests are intended to call out the oblivious, not to really trick you.
I've received exactly one actual phishing email (not a test) in my career. Others may have been sent but filtered out before reaching my inbox.
