This 118-bed facility that opened in 1987 embraces the concept of resident-centered care, where nurses and assistants provide 24-hour hands-on individualized care with a team approach. Services include physical, speech, and occupational therapy, and hospice and respite care are available. Shepherd’s Choice is also a secured facility that uses a watch-mate system to protect residents who suffer from Alzheimer’s or dementia-related diseases. At-risk patients wear a bracelet that triggers the outside doors to lock or alarms to sound if the patient begins to wander away. Residents’ rooms feature floor-to-ceiling windows, providing an opportunity to watch deer and other wild animals, and clients are encouraged to personalize their rooms. The center provides dental and optometry services on-site, and it has a beauty/barber shop. Planned group and individualized activities are also available. This nonprofit skilled nursing facility accepts payment for its services from Medicare, Medicaid, private insurance, and private pay.
