Observation:Has bare wood shelving on rack used for food storage in dry storage area for kitchen, must seal with at least two coats of light colored paint or varnish to make smooth, cleanable, and nonabsorbent, to aid cleaning.

Observation:Need handwashing signage posted in RR's used by kitchen employees, given examples today.
- Storage of clean linens, single-service, and single use articles
Observation:Case of single use cups on floor in dry storage area for foods, must store 6" off floor.
- Sanitization methods - hot water, chemical
Observation:Was using commercial general disinfectant on food contact surfaces, must use product that is approved for food contact surfaces, such as chlorine, quat or iodine. Does have bleach available, and will use per instructions for 50-100 ppm.
- Food storage - preventing contamination from the premises
Observation:Shelving rack for food in storage area for kitchen is set up directly on floor, must adjust lowest shelf to be at least 6 inches off floor to aid cleaning.
- Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
Observation:Must have all current and future employees complete the employee reporting of illness form, and keep on file at business. Given copy today.
- License required to operate a food establishment
Observation: Took over ownership of hotel with breakfast service without obtaining a valid IA Food Service license. Must pay double fee.
- Food employees hair is effectively restrained
Observation:Staff working with breakfast area must wear hat, visor, or net to properly restrain hair.
