    Observation: Facility does not have required written procedures for clean up of vomiting and diarrhea. Provided Noro guidance document.
  • Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
    Observation: Facility is not using Employee Health reporting Form 1B.

  • Manual and mechanical warewashing equipment, chemical sanitation - temp, pH, concentration, and hardness
    Observation: The facility was not sanitizing in their third hole of their sink.

    Observation: The sink did not have a hand wash sign placed by it.

    Observation: The facility does not have a diarrhea vomit clean up plan. I provided a noro clean up plan and the facility stated they would copy it and place it into protocols.

    Observation: Packaged food did not have an ingredient list, net weight, name of the product, and address where th product was made.
  • A temperature measuring device with a suitable small diameter probe is provided and readily accessible to measure the temperature of thin foods
    Observation: The facility does not have a thin tipped thermometer.
  • The ability to explain the responsibility of the person in charge for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease.
    Observation: The facility does not have a sick policy.
  • Certified Food Protection Manager with management and supervisory responsibility employed by the establishment
    Observation: The facility does not have a certified food protection manager.
