Observation: There was a spray bottle of bleach that exceeded the required concentration.
- Physical facilities maintained in good repair
Observation: The caulking at the kitchen handsink was in bad repair. The top cover of the ice machine was in bad repair
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation: There was no thermometer placed near the milk and deli meat display doors of the walk-in cooler.
- Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
Observation: There was raw bacon and eggs stored over RTE food items at the display door.
- Storage of clean linens, single-service, and single use articles
Observation: There were pizza boxes in the kitchen and single serve cups in the backroom that were stored on the floor.
- A temperature measuring device with a suitable small diameter probe is provided and readily accessible to measure the temperature of thin foods
Observation: Establishment did not have a thin tipped thermometer.

Observation: The lights in the backroom above the ice machine were not properly shielded.

Observation: There were containers of bleach stored over "to go" containers in the back room
- Ventilation hood system and filters designed to be cleanable, prevent dripping, and are adequate to prevent grease or condensation from collecting on walls and ceilings
Observation: The hood filters were soiled.

Observation: In general, the floor was soiled along the walls and along equipment. The caulking at the kitchen handsink was in bad repair.

Observation: There was not a thermometer placed near the milk/sandwich display doors in the walk-in cooler.

Observation: The fan guards in the in the walk-in cooler were dusty. The hood was soiled. The exteriors/handles of equipment were soiled.- walk-in cooler door handles. The pop nozzles/ice chute were soiled.

Observation:There was floor cleaner and hand soap stored above single service-ware in the backroom. PIC corrected at inspection.
