Observation: There is no hand sink behind the bar downstairs. COS by designating one compartment of the three compartment sink to be a hand washing sink only and the other two compartments will be utilized for wash, rinse, and sanitize for bar glasses. Establishment has six months from today's date (07/11/2016) to place a separate hand sink other than using the existing three compartment sink.

Observation: There were working container spray bottles of sanitizer and table cleaner that were not identified with the common name. COS by labeling.

Observation: There are no test strips to check the concentration level of the quat based sanitizer and the chlorine sanitizer for the warewash machine.

Observation: There is no designated hand sink behind the downstairs bar.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation: There are multiple refrigerator/freezers that do not have thermometers in them.
- Manual and mechanical warewashing equipment, chemical sanitation - temp, pH, concentration, and hardness
Observation: The sanitizer in the spray bottle in the kitchen used for food contact surfaces registered 500+ ppm for concentration level. COS by discarding and achieving correct concentration level of sanitizer.
