Observation: The facility does not have a certified food protection manager.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation: The chest fridge did not have a thermometer placed inside the unit.

Observation: The ice machine was not clean to sight. The person in charge cleaned the unit prior to me leaving.

Observation: The facility does not have a diarrhea vomit clean up plan. I handed out a noro clean up plan to the person in charge and he stated we would copy it and place it into policy.

Observation: The facility does not have a set of test strips to test the sanitizer with.
- The ability to explain the responsibility of the person in charge for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease.
Observation: The person in charge was unsure about a sick policy on hand. I handed out a sick policy and went over the policy with the policy in charge.
- Manual and mechanical warewashing equipment, chemical sanitation - temp, pH, concentration, and hardness
Observation: The Person in charge was not sanitizing in the third hole of the sink. I told the person in charge to place sanitizer in the third hole of the sink and he stated he would.

- Duties of PIC
The facility had a sick policy but the person in charge did not remeber the symptoms and the food borne illnesses. The person in charge did not remeber signing the policy.
- Food temperature measuring devices are provided and readily accessible
The facility did not have a thin probe thermometer to check the food with. The fridge units did not have a thermometer placed in them.
- Posting of a valid license
- Linens- cleaning and storage
Straws are not being protected in the front prep area.
