Observation: Wiping cloths were laying on the counter tops after each use.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation: Cooked noodles and rice were not date marked. The units were dated prior to me leaving.
- Ventilation hood system and filters designed to be cleanable, prevent dripping, and are adequate to prevent grease or condensation from collecting on walls and ceilings
Observation: The vent hood above the oven was missing a vent filter.

Observation: One drinking cup was uncovered in the food prep area. The unit was removed.

Observation: Dish racks were setting on the floor in the dish washing area.

- Established procedures for responding to vomiting and diarrheal events
Observation: The facility does not have a diarrhea vomit clean up plan in place. I provided them with a noro clean up document and the PIC stated they would copy it and place it into protocol.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: Non food contact surfaces were not clean to sight.

Observation: The sinks did not have any paper towels.
- Disclosure of menu items offered or served raw or undercooked
Observation: If salmon is going to be under cooked there needs to be a disclosure on the menu and it has to be asterisked.
- Handwashing signage
Observation: The sinks did not have hand wash signs placed by them.
- equipment food contact surfaces and utensils clean to sight and touch.
Observation: The food contact surfaces were dusty.
- Food storage containers identified with common name of food
Observation: Several containers were not labeled.
- The ability to explain the responsibility of the person in charge for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease.
Observation: The facility does not have a sick policy in place. I provided a sick policy and the PIC stated they would place it into protocol.
- Multiuse food contact surfaces are cleanable
Observation: The cutting boards are not cleanable and shall be discarded or resurfaced. Steel sponges were not clean to sight.
