Observation: The facility does not have a thin tipped thermometer.

Observation: The ice chute was not clean to sight. The facility was cleaning it prior to me cleaning.

Observation: The facility did not have the employees sign sick policies. I provided the PIC with a sick policy and she stated they would have all employees sign the sick policy.

Observation: The front hand sink did not have a hand wash sign posted by it. I provided them with one and they posted it.
- Cold and hot holding equipment used for PHF/TCS foods are equipped with at least one temperature measuring device that measures temperature in the coldest part of a hot holding unit or the warmest part of a cold holding unit and located and designed to allow for easy viewing.
Observation:The front fridge unit does not have a thermometer placed in it.

Observation: The person in charge did not know what the 5 food borne illnesses and symptoms were.

Observation: Single use coffee cups were out of protection.

Observation: The facility did not have a procedure for cleaning vomiting and diarrhea events.

Observation: The sanitizer was 100 ppm. The quat shall maintain 200 ppm.

- The ability to explain the responsibility of the person in charge for preventing the transmission of foodborne disease by a food employee who has a disease or medical condition that may cause foodborne disease.
The person in charge did not have a sick policy on hand and did not know the 5 symptoms of food borne illness or the 5 food borne illnesses. I handed out the policy and went over the policy with the manager and staff. The manager had everyone sign the policy who was working and would make sure everyone else would sign the policy the next day. Corrected at time of inspection

A couple of shaker containers in the prep area were not labeled.

The quat sanitizer was 100 ppm. The concentration shall maintain 200 ppm. Corrected at time of inspection
