Observation: Dressing and sauce containers on prep table and on top of reach-in cooler in kitchen were not labeled. All containers holding food which are not unmistakably recognized shall be marked with the common name of the food.

Observation: Food storage boxes in contact with excessive ice build up in white chest freezer. Packaged food cannot be in direct contact with ice or water.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation: Sausage gravy was not date marked. If food remains on premises for period longer than 24 hours after opening it must be date marked and discarded after 7 days time. Gravy was discarded at time of inspection.
- Handwashing signage
Observation: Men's restroom hand-washing sign had been ripped off the wall. All hand-washing sinks used by employees shall have a sign which notifies employees to wash their hands.

Observation: License for establishment was not posted. All food establishments must post license in conspicuous location.
- Posting inspection reports
Observation: Previous inspection reports were not posted. All food establishments must post most recent inspection report in conspicuous location.
- Equipment location, installation, repair, and adjustment
Observation: Doors to walk in cooler and walk in freezer did not close on their own. All cooler and freezer doors must be self closing and fit tight.
- Food storage - preventing contamination from the premises
Observation: Food was being stored on the floor in both walk in cooler and the walk in freezer. All food must be stored at least 6 inches above the floor to prevent exposure to splash, dust, or other contamination.
- Multiuse food contact surfaces are cleanable
Observation: 2 Tupperware containers were beginning to form holes on sides. All food containers must be smooth, easily cleanable, and non absorbent. Containers were discarded during time of inspection.

Observation: Provide cove base for 1 ) kitchen area, 2) Side wall next to drink dispensers in retail area near kitchen 3) Entrance area to restrooms.

Observation: Soap dispensers need to be installed in both men and women's restroom.
- Plumbing system repaired according to law
Observation: Provide a food preparation sink to wash vegetables for pizzas. Ensure a 1 inch air gap is provided in drain line.
- Material characteristics of non-food contact surfaces
Observation: 1) Caulk around both hand sinks in restrooms. 2) Cupboard doors under pop machine are warped and no longer cleanable. Having doors replaced this week.

Observation: Provide a self-closure for back door in large storage room.
