Eating, drinking, or using tobacco Observation: There was an ashtray with a cigarette is the back room where monitor for the surveillance cameras are located. An employee shall eat,drink, or use any form of tobacco only in a designated area. Smoking indoors of the establishment is strictly prohibited according to the clean air act of Iowa.
Storage and maintenance of wet and dry wiping cloths Observation: Wet wiping cloths on and behind the bar. All wiping clothes must be stored in sanitizer or hung to air dry.
Physical facilities maintained in good repair Observation: Drip tray at wash sink behind bar has paint pealing and is rusty. All surfaces must be smooth easily cleanable and non absorbent. Vents on cooler fans in walk in cooler soiled. Clean as often as necessary to keep clean.
Established procedures for responding to vomiting and diarrheal events Observation: The establishment does not currently have a noro clean up procedure although they have the paperwork that they needed one from the last inspection.
Test kit provided and used to measure sanitizing solution concentration Observation: Test strips were expired that were available and had turned yellow. A proper testing kit must be provided to test that the sanitizer solution is maintained at 200 to 400 ppm for a quat sanitizer.
Certified Food Protection Manager with management and supervisory responsibility employed by the establishment Observation: No certified food protection manager employed on site. A certified food protection manager must be employed by the establishment according to the food code changes that were adopted 1/1/2014.
Storage or display of food in contact with water or ice Observation: Excessive frost buildup in food storage freezers in the pool table room. Defrost the freezers. All food contact surfaces must be clean to sight and touch. Clean as often as necessary to keep clean.

Drying mops Observation: Used mop was stored in mop container. Must provide a hook for mop or drape over bucket to allow to air dry in between uses.
Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods Observation: Eggs were stored above ready-to-eat sauce containers in white refrigerator in kitchen. Must be stored by the cook temperature: higher cook temp items must be stored on the lower shelf.
Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean Observation: Racks in walk-in cooler have food buildup. Clean more frequently to prevent contamination.
Ventilation hood system and filters designed to be cleanable, prevent dripping, and are adequate to prevent grease or condensation from collecting on walls and ceilings Observation: Vents are dusty in walk-in cooler. Must clean off dust buildup on vents as needed.
Equipment location, installation, repair, and adjustment Observation: Gasket on white refrigerator is torn in kitchen. Replace as needed.
Light bulbs, protective shielding Observation: Light bulb was not covered or shielded in white refrigerator. Must provide an adequate cover or shatter resistant bulbs in area where there is exposed food.
