Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A) Observation: No verifiable Employee Health Reporting Agreement available at time of inspection. Inspector provided person in charge a copy of the FDA form 1B Reporting Agreement. Per person in charge all food employees will read and sign the agreement.
Bare hand contact with ready to eat foods Observation: Food worker observed adding cheese and cut vegetables to a cooked pizza without wearing gloves. Pizza was topped with ready to eat garnishes prior to being placed at the service counter. Inspector discussed the violation with employee. Employee washed hands and put on single service gloves. Food worker used bare hand contact to remove pizza slices out of the oven on the service line and on to customers to go plates. Inspector discussed violation with person in charge. Manager placed tongs on the service line for staff to handle transferring pizza slices.
Eating, drinking, or using tobacco Observation: Open employee drink with no lid stored on the shelf above the pizza prep cooler in the kitchen, and on top of the salad prep cooler on the front line. Both drinks were removed from the kitchen by employees.

Common name-working containers Observation: multi-purpose chemical spray bottles were not labeled with the name of contents. Corrected by staff.
Cooling foods are stored properly Observation: Fresh grilled red bell peppers were cooling on the counter with tight fitting plastic wrap. Employee removed the wrap to allow rapid cooling.
Food employees hair is effectively restrained Observation: Employee working with and handling food were not wearing any kind of hair restraint.
