Observation: Banquet hall bar didn't have a hand drying method accessible. Must provide paper towels at hand sink to prevent contamination. REPEAT VIOLATION

Observation: Food debris buildup inside of the Men's reach-in freezer. Clean more frequently.
- Cleanability of floors, walls, and ceilings
Observation: 1) Damaged wall in Men's restroom. Fix 2) Missing flooring tiles underneath three compartment sink at main bar. REPEAT VIOLATION. Repair. Must provide a smooth, easily cleanable, and non-absorbent surfaces.
- Design and construction of equipment including
hot oil filtering equipment, can openers, non-food contact surfaces, kick plates, ventilation hood systems filters, bearings and gear boxes, and condenser units

Observation: Didn't provide hand soap at banquet hall bar.
- Cleaning frequency of food contact surfaces (PHF/TCS)
Observation: 1) Food debris buildup inside of pizza oven. Clean more frequently to prevent contamination. 2) Visible buildup on can opener. Clean after each event. 3) Mold buildup on inside of ice machine. Must empty, wash, rinse. and sanitize before allowing to fill up again. REPEAT VIOLATION
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Observation: A gallon milk was not dated in reach-in cooler. Must date mark all ready to eat food with date opened prepared and then discard within 7 days unless entire contents will be used within 24 hours. Put a note on fridge door. REPEAT VIOLATION
- Miscellaneous sources of contamination
Observation: Ice bins at both bars were not covered. Must cover all food items when not in immediate use to prevent contamination. REPEAT VIOLATION
