Observation: Missing coving/base board tiles in areas of the kitchen. Water damaged ceiling tiles in back kitchen area and some ceiling tiles missing.

Observation:Handle on the food dicer is repaired with duct tape.

Observation: Ingredients stored in containers without a label indicating the name of the contents (stored above the pizza cutting prep table).

Observation:Wet mops are stored on the floor and in the mop sink and are not air dried in a manner to prevent contamination of other surfaces.
- Non-food contact surfaces and food contact surfaces of cooking equipment and pans are clean
Observation: Ice machine water discharge tubing has an accumulation of black residue inside the tubing.

Observation:The paper towel dispenser was empty at the handsink by the 3 compartment sink. Employee filled the paper towel dispenser during the inspection.
- Food employees hair is effectively restrained
Observation:Food handlers wearing inadequate hair restraints (visors) that do not enclose the hair on top of the head.
- In-use utensils, between-use storage
Observation: Cups used as scoops are stored in food products with the handle in contact with the food product.
- Eating, drinking, or using tobacco
Observation:Food handler drank from an open beverage container that was stored on the prep table where pizza boxes are stored. Food Code 2-401.11 Eating, Drinking, or Using Tobacco.(A) Except as specified in ¶ (B) of this section, an EMPLOYEE shall eat, drink, or use any form of tobacco only in designated areas where the contamination of exposed FOOD
- equipment food contact surfaces and utensils clean to sight and touch.
Observation:Food dicer soiled with pieces of dried food particles stored on box of solo drink cups. Manager stated that it is broken and they haven't used it for weeks. Equipment was washed during the inspection. Interior of ice machine soiled with black residue. Ice was emptied out of the ice machine during the inspection and process of cleaning the interior of the ice machine was begun during the inspection. Can opener blade was dirty while not in use. During the inspection the can opener blade was placed in the 3 compartment sink to be washed.
- Hand and arm jewelry prohibition
Observation:Food handler wearing a watch on their wrist during food prep activities.
- Package integrity
Observation:Severely dented can of pizza sauce with depressions in the top of lid and along the seams stored on the can rack. During the inspection, the manager placed the dented can in another location for disposal or return to supplier.
- Sponges-use limitations
Observation:Sponges are used to wash food contact surfaces.

Observation:Spray bottle of glass cleaner stored on surface with cardboard food containers. Chemicals stored above ice machine and above single service food containers in the back room. All chemicals were moved below and away from food and food contact surfaces during the inspection.
