    Observation:There is no thermometer in the white kitchen refrigerator - corrected on site.
  • Food storage - preventing contamination from the premises
    Observation:Observed uncovered pie cooling on countertop at front wait station counter, corrected on site.
  • Disclosure of menu items offered or served raw or undercooked
    Observation:Menu items served undercooked (eggs, hamburgers) are not marked with a star or asterisk that corresponds to the consumer advisory reminder statement at the bottom of the 2nd page. Corrected on site.
  • Storage of clean equipment and utensils
    Observation:Observed clean utensils stored on the ledge behind the handsink faucet and hanging on hooks above the sinks, in an area prone to splashing by hand washing.
  • PHF/TCS food that is cooked and cooled on premises is reheated within 2 hours to 165°F or above for 15 seconds for hot holding
    Observation:Observed soups reheating in crockpot warmer - soups were 58 and 62 degrees after 30 minutes. Soups transferred to stovetop, corrected on site.

    Observation:Observed leaking drain under sink at front counter.

    Observation:Observed lids for food containers stored in cardboard box that has grease stains, instead of a durable washable container. Observed greasy, soiled cloth on top of mixer bowl in pizza prep kitchen. Observed unsealed, unpainted wood on countertop under pizza oven.
  • Using a handwashing sink- operation and maintenance
    Observation:Observed employee cleaning thermometer at the kitchen handsink, corrected on site.
  • Manual and mechanical warewashing equipment, chemical sanitation - temp, pH, concentration, and hardness
    Observation:There was no detectable level of sanitizer in dishwashing machine - chemical in container appeared to be old and no longer at correct concentration. A new container was added, feed line was primed, corrected on site.
  • Insect control devices are properly designed and installed
    Observation:Observed fly swatter in kitchen.
  • Equipment location, installation, repair, and adjustment
    Observation:Observed pop syrup leaked onto countertop under the fountain pop machine at front counter.
