All food was discarded. Cooler may not be used to store potentially hazardous foods till it is repaired or replaced.

Cleaning frequency of food contact surfaces (non-PHF/TCS) Observation: Maximum reach-in freezer has food debris on the bottom of shelving. Clean as needed.
Heating, ventilating, and air conditioning systems design, installation, and cleaning Observation: Dusty fan screen covers in walk-in cooler and freezer. Clean more frequently to prevent contamination.
Drying mops Observation: Mop sitting in bucket. Must provide a hook for mop or drape over bucket to allow to air dry in between uses.
Physical facilities maintained in good repair Observation: Observed burnt out light in grill/oven area and above self-serve counter. Replace.
Sanitization methods - hot water, chemical Observation: Didn't have sanitizer available at time of inspection to clean food counter top and tables in dinning-in room. Run out QA sanitizer solution. Must have sanitizer made during all business hours to prevent buildup of bacteria. Using chlorine sanitizer solution from dish machine.
Food is properly labeled Observation: Self serve cookies didn't have an ingredients. Must provide an ingredients label for all self-serve food can post labels in cookies or keep a booklet near register and post a sign on cookies table stating "ask for ingredients"
Discarding ready to eat, PHF/TCS food prepared on-site or opened commercial container held at 41°F for 7 days Observation: Observed food out of date in reach-in cooler on self-serve bar. Food must be used or discarded by date marked on food.

Handwashing signage Observation: Didn't post handwashing sign on hand sink in kitchen. Every hand sink to be used by employees must has sign posted reminding employees to wash their hands.
