Failure to store raw materials in a manner that protects against contamination. Specifically, packaged raw meat was being stored on the top shelf of the refrigerator and in contact with and above other non-meat ingredients.
- Self-closing doors
Toilet facilities lack self-closing doors. Specifically, the two restrooms on the main floor do not have self-closing doors.
- Storage of personal items
Personal belongings were stored in an area where food is exposed. Specifically, an uncovered drink and a cell phone were on the counter in the bakery. Refrigerator being used for personal items was not labeled as such and was not separate from the production area.
- Storage/transportation of finished goods (contamination)
Failure to store finished food under conditions that would protect against physical and chemical contamination. Specifically, uncovered bread was in contact with a decorative doily on bare wood shelving. A bottle of Fantasik spray was sitting on the floor near uncovered bread. The sprayer of the bottle was near and pointing in the direction of the bread.
- Suitable locations
Failure to provide hand washing facilities at each location in the plant where needed. Specifically, there is not a hand sink located in the converted garage (bakery)
- Hand drying
Lack of a sanitary towel service or suitable hand drying devices. Specifically, a cloth towel is being used for hand drying at the sink used for hand washing.

- Floors, walls and ceilings
The plant is not constructed in such a manner as to allow the ceiling to be adequately cleaned and kept clean. Specifically, the ceiling is unfinished drywall.
- Failure to clean - general
Failure to clean food-contact surfaces as frequently as necessary to protect against contamination of food. Specifically, two mixing bowls and mixing attachments had not been cleaned after previous use and were observed to be soiled.
