Observation: Several raw meat boxes were stored on the floor in walk-in cooler. Food must be 6" off the floor to prevent contamination and cleaning purposes.

Observation: Hot water handle on mop sink leaks. Must repair or replace to maintain in good condition. REPEAT VIOLATION.

1) Backflow prevention device on mop sink faucet leaks. 2) Hot water handle on mop sink leaks. 3) Faucet on 3 compartment sink leaks. Must repair or replace to maintain in good condition.
- Date marking for ready to eat, PHF/TCS food prepared on-site or opened commercial container held for more than 24 hours
Overall have great date marking. Two bottles of milk in reach-in cooler were not date marked. Must date mark all ready to eat food if not used within 24hr after opened or preparation. Have 7 days from date marking to use or discard. Corrected at time of inspection
- Separating raw animal foods from raw ready to eat food and separating raw animal food from cooked ready to eat foods
Raw chicken was stored above ready to eat food in reach-in cooler. Raw animal food must be separated or stored below from ready to eat food. Corrected at time of inspection

1) Cabinet by white refrigerator in kitchen has bare wood drawers. 2) Cabinet by ATM machine in dining area has bare wood surface. 3) Cabinet across walk-in cooler in kitchen, wood surface has paint chipping away. Must paint with at least two coats to make smooth and easily cleanable.
