Observation: The quat sanitizer was 400 ppm. The person in charge remixed the sanitizer and it was 200 ppm.

Observation: The facility does not have a diarrhea vomit clean up plan. I provided a noro clean up document and the person in charge stated he would place it into protocol.

Observation: The facility does not have test strips to test quat sanitizer.

Observation: Single use cups in the front bar area were not protected with a plastic sleeve or a dispenser unit.
