Observation: Several shelves had crumbs on them.

Observation: Scoops in the sugar and flour containers were laying with handle in the food. The units were propped up.

Observation: Lettuce was being self served while not being stored under the sneeze guard. The unit was assigned to be served by a cook.

Observation: The facility does not have a certified food protection manager on site. Will check this one in the spring.

Observation: The facility die not have a written diarrhea vomit clean up plan. I provided the PIC with a noro clean up document and she stated she will copy it and place it into protocol.

Observation: The staff does not have sick policies that have been signed. I provided a sick policy to the PIC and she stated she will make sure everyone signs the sick policy.

Observation: One bucket of quat test 0 ppm. The bucket was dumped and remixed.

Observation: The sneeze guard and the floor container in the dry storage room were damaged.

Observation: The facility does not have a written procedure for cleaning up vomit and diarrhea. I handed out the state clean up procedure for cleaning up diarrhea and vomit. The facility was creating their own when I left.

Observation: The test strips that were being used appeared to be out of date.

Observation: The bleach water was 0-10 ppm. The facility remixed it to 50 ppm.
