    Observation: Lights in the prep room are not shielded.
  • Bare hand contact with ready to eat foods
    Observation: Bare hand contact with deli meats and cheeses. No bare hand contact was discussed with person in charge.
  • Using a handwashing sink- operation and maintenance
    Observation: Handsink is soiled.
  • Material characteristics of non-food contact surfaces
    Observation: Cardboard box used to store utensils in meat dept.
  • Cutting surfaces maintained
    Observation: Cutting boards in meat dept have a large accumulation of deep, discolored scratches.
  • Cleanability of floors, walls, and ceilings
    Observation: Walls in the back storage area are Styrofoam insulation making them not easily cleanable. Walls in meat dept and doors to coolers are rusted make them difficult to clean.
  • Physical facilities maintained in good repair
    Observation: Walls behind meat display cooler are damaged.
  • Food storage - preventing contamination from the premises
    Observation: Food stored on the floors of the walk in cooler and the meat dept walk in cooler.
  • Removing dead or trapped birds, insects, rodents, and other pests
    Observation: Fly paper strip over the warewashing area with a large accumulation of dead insects.
  • Physical facilities maintained clean: Cleaning frequency, restrictions, and methods
    Observation: Display coolers soiled. Large amount of ice build up in the meat display cooler. Fan covers in the meat dept walk in soiled. Walls in the meat dept soiled.
  • Food employees and conditional employees are informed in a verifiable manner of their responsibility to report in accordance with law, to the person in charge, information about their health and activities as they relate to diseases that are transmissible through food, as specified under 2-201.11(A)
    Observation: No verifiable manner to determine if food employees are aware of reporting responsibility.
