In the frigidaire reach in an open package of deli ham was dated for 11/3/15 and there was a container labeled canned tomatoes with a date of 10/6/15. COS-ham was discarded. KDA_3_50118A2 - --In the walk in cooler, a container of made on site gravy did not have a date mark on it. Person in charge said it was used for Thanksgiving meal. In the frigidaire reach in, an opened package of deli smoked turkey breast was with out a date marking. Person in charge did not know when it was opened. COS-this items were discarded. KDA_4_20311 - --Person in charge could not locate food thermometers. KDA_4_204112B - --There was not a thermometer in the frigidaire reach in. KDA_4_50111A - --The frigidaire reach in is missing the handle. The drawer on the interior bottom is missing a big piece out of it. KDA_4_60111A - --Located on the knife strip, there were three knives that had dried on food debris on the cutting blade. Located on the make table, a pizza cutter had dried on food debris. COS-These items were placed at the three compartment sink. KDA_4_60111C - --The bottom of the vent hood, has an accumulation of grease. The front side of the freezer reach in and frigidaire reach in has an accumulation of food debris. KDA_4_60212B - --The microwave, had a heavy accumulation of dried on food debris on the interior and the front exterior and handle. KDA_4_60213 - --The stove, has an accumulation of grease, dried on debris on the fronts, sides and the panel that has the control knobs. The frigidaire reach in has food debris on the floor. KDA_4_90311A - --Located next to ice machine, a box of a 1000 count single serve and wrapped straws were sitting on the floor. COS-straws were moved off floor. KDA_5_20511A - --There was a soiled towel in the basin of the hand washing sink. COS-towel was removed. KDA_6_20211A - --The light bulbs in the bar area, are not shatter proof and are not covered. KDA_6_20215A3 - --The front door has a 1/4" gap along the bottom right side. KDA_6_30111 - --There was not hand cleaning soap at the hand washing sink. COS-hand washing soap was taken from the three compartment sink in the bar area and placed at hand washing sink. KDA_6_30112 - --The hand washing sink did not have hand drying towels. COS-hand drying towels were placed at the sink. KDA_6_50111 - --In the gaming area, part of the baseboard is missing. The baseboard around the bar area, is missing in areas. There is one broken ceiling tile to the left of the stove. There is broken and one ceiling tile that is very loose and hanging down in front of the walk in cooler. KDA_6_50112A - --Ceiling tile and the vent cover to the right of the grill has an accumulation of dust. There is dirt and debris and a cigarette butt located around the hot water heater. There is dirt and debris around the reach in freezer and cooler. KDA_6_50119 - --The women's restroom door was propped open with a garbage can during business hours. COS-door was shut. KDA_7_10211 - --Sitting in the bar area, a spray bottle with a blue liquid in it was unlabeled. Person in charge said it was 409. COS-bottle was labeled. KDA_7_20111A - --There was an open bottle of Palmolive sitting on top of the hot heater. The hot water heater is located next to the stove. On top of the stove top were cleaned pots. COS-Palmolive was moved. KDA_Defacto - --Frozen food is frozen. --There is not a mop sink, they fill the mop bucket with a hose hooked to the water line. They dump water down the drain in the floor or outside. --Walk in cooler, gravy 38.2F. Frigidaire reach in, sliced yellow cheese 39.7F.	12/03/2015	21	Regular	Out
KDA_3_50118A2 - --In the reach in cooler, there was an opened packaged of commercially prepared deli style sliced smoked honey ham and an open package of deli sliced smoked honey turkey. Person in charge said it was opened yesterday (12/15/15) around noon. COS (corrected on site)-lunch meat was dated with correct date. KDA_4_204112B - --The reach in refrigerator does not have a thermometer. KDA_4_50111A - --The frigidaire reach in is missing the handle. There is no drawer on the bottom left in the frigidaire reach in cooler. KDA_4_60111A - --Located above the three compartment sink, two knives were stored as clean on a knife strip with grease and dried on food debris. COS-knives were placed at three compartment sink. KDA_4_60212B - --The microwave, had a heavy accumulation of dried on food debris on the interior ceiling of the microwave. KDA_6_20211A - --The light bulbs in the bar area, are not shatter proof and are not covered. KDA_6_20215A3 - --The front door has a 1/4" gap along the bottom right side.
