KDA_2_40111 - --Employee at the counter was chewing gum at during the inspection. COS Educated the employee and the employee threw the gum away.
KDA_3_10111 - --There was a container of fresh strawberries being stored on the shelf in the bottom portion of the MT that were rotten and had white mold growing on them. COS Item was discarded.
KDA_3_30212 - --There was a squeeze bottle with a yellow liquid being stored in it on the counter in the kitchen area. Employee said it was olive oil. COS Bottle was marked.
KDA_5_20511B - --There was three sink drain plugs, a wet paper towel, and a funnel blocking the hand sink drain and was not able to wash hands. COS Items were moved.
KDA_5_50117 - --There is no cover on the trash receptacle located in the womens restroom.
KDA_6_30111 - --There was no hand soap at the hand sink located in the kitchen area. COS Hand soap was placed at the hand sink.
KDA_6_30112 - --There were no paper towels at the hand sink located in the kitchen area , and in the mens restroom. COS Paper towels were placed at the hand sinks.
KDA_6_30114 - --There was no hand washing signage posted at the hand sink located in the kitchen area, mens, and womens restrooms. COS Hand washing signage was posted.
KDA_6_50111 - --There is a hole in the wall next to the mop sink located in the kitchen area.
KDA_7_10211 - --There was a working spray bottle with a purple liquid being stored on top of the trash bin located in the customer area with no common name on it. Employee said it was glass cleaner. COS Chemical was marked. There was a bucket of clear water with towels in it and a bucket of blue water with a sponge in it located at the three vat sink with no common name on the containers. Employee said it was bleach water and soapy water. COS Chemicals were marked.
KDA_7_20111A - --There was a container mop an glow, multi surface cleaner, and sani wipes being stored on a shelf directly next to a crock pot located in the kitchen area. COS Chemical was moved.
