KDA_3_30212 - --There were containers of dried mint, potato flakes and various spices in the back kitchen area without a common name listed on the labels. PIC identified all the above mentioned products.
KDA_3_50116A2 - --There was a container of cooked chicken in the reach in cooler on the prep line that measured at 45F. The ambient temperature measured at 40F and all other potentially hazardous products in the cooler measured at 41F or lower. PIC stated the chicken was moved from the walk in cooler 30 minutes prior. Discussed importance of the chicken being placed under temperature control immediately when moved with the PIC. COS- PIC moved the chicken back to the cooler for additional cooling.
KDA_3_60311A - --There is no disclosure or reminder for undercooked foods on the menu or in the establishment. PIC stated hamburgers can be cooked to order. COS- inspector provided temporary template for a consumer advisory.
KDA_4_20211A2 - --There was a rubber spatula on the clean utensil rack with a broken and chipped food contact surface. COS- PIC voluntarily discarded.
KDA_4_50111B - --The lid to the first make table on the prep line is broken on one side.
KDA_4_60111A - --There was old food/grease debris along the inside portion of the industrial mixer bowl. COS- PIC moved the bowl to an area for additional cleaning. There was a pair of tongs with old food debris along the food contract surface hanging on the clean utensil rack. COS- PIC moved the tongs back to the dishmachine for additional cleaning. There were approximately 5-6 plastic/metal food storage containers on the clean utensil rack with old date sticker/date stick residue along the sides. COS- PIC moved all the containers back to the dishmachine for additional cleaning.
KDA_4_60111C - --There is dust and rust build up on the fan guards in the walk in cooler.
KDA_4_60211E4 - --There was mold build up along the upper inside portion of the bulk ice machine in the kitchen.
KDA_6_50112A - --There was standing gray water in the mop sink. There was grease and old food debris behind the fryers. There was dirt and grime build up throughout the facility.
KDA_6_50116 - --There was a mop with a handle stored in the mop bucket not allowed to properly air dry.

PIC stated the above mentioned foods were either made yesterday 11/3 evening or today 11/4. Discussed date marking with PIC. There was some date marking through the facility.
