- 5-202.14 - --There is hose with sprayer attached directly to the water line under the dishmachine without any backflow prevention devices attached.
- 3-501.17(A) - --There were containers of lasagna mix, chicken, grape leaves and cooked eggplant in the walk in cooler without any date marking. PIC stated all previously mentioned items were removed from the freezer on Saturday 12/19. COS- educated staff on proper date marking and PIC correctly date marked.
- KDA_2_30114F - --An employee handled a visibly soiled rag with gloved hands and then handled a RTE cooked tortilla with the same gloved hands. COS- educated staff on hand washing procedures, PIC voluntarily discarded the tortilla and the employed washed hands before donning new gloves.
- KDA_3_10111 - --There was an opened gallon of milk in the walk in cooler without any date marking. PIC was unable to identify the open date. COS- PIC voluntarily discarded the remaining milk.
- KDA_3_20215 - --There was a 7lb can of Pimientos and a 7lb can of Tomato ketchup on the dry storage rack in the back with large dents along the seam compromising the seal. COS- PIC moved both cans to another area for returns.
- KDA_3_30212 - --There were containers of unidentifiable dry goods on the shelf in the prep area without a common name listed on the container. PIC identified the products as sugar, flour, breading, etc.
- KDA_3_30412F - --There was a knife used for cutting raw salmon stored in a QT solution at the front counter. Educated on proper utensil storage.
- KDA_3_30414D - --There were several dry wiping cloths out on the cook line that were visibly soiled. PIC stated the cloths were used to wipe off the food contact counters.
- KDA_3_50113B - --There was calamari thawing in the handsink in the back.
- KDA_3_50114A2 - --There was a container of cooked tomato sauce (made on site) in the walk in cooler that measured at 51F. PIC stated the sauce was made yesterday 12/20 and was not pulled out today 12/21. COS- PIC voluntarily discarded the sauce.
- KDA_3_50116A2 - --There was a container of approximately 20 butter packets measuring at 68F and an opened container of half/half measuring at 67F on an ice hold at the front counter. PIC stated both items were removed from the cooler approximately 2 hours prior. COS- PIC voluntarily discarded both items per his own discretion.
- KDA_4_60111A - --There were approximately 20-30 plastic food storage containers on the clean utensil shelf with old date sticker residue along the side. There was old food debris build up on several serving utensils, tongs, and dicers on the clean utensil shelf. There was old food debris along the food contact blade of the commercial opener. COS- PIC moved all previously mentioned items to the warewashing areas for additional cleaning.
- KDA_5_20314 - or, Installing an APPROVED backflow prevention device as specified under § 5-202.14.
- KDA_5_20511A - --There was an industrial soup kettle unit placed in front of the only handsink in the back prep area making the handsink inaccessible to staff. COS- PIC moved the kettle to a different area.
- KDA_5_20511B - --There was a container of calamari thawing in the back handsink. COS- PIC moved the container back to the walk in cooler. There were approximately 3 dirty drinking glasses in the basin of the bar handsink. COS- PIC moved the glasses to the warewashing sink.
- KDA_5_40211 - --There was calamari thawing in the basin of the handsink in the back prep area that is directly plumbed.
- KDA_6_20211A - --There was no protective shielding over the lights above the prep area or the lights above the dry goods storage area.
- KDA_6_501110B - --There were several employee purses and jackets hanging on the dry goods shelves in the back room directly above food and clean utensils.
- KDA_7_10211 - --There was a container of blue liquid on the ground under the make table at the front without a common name listed on the label. PIC identified the product as sanitizer water. COS- PIC correctly labeled.
- KDA_7_20111B - --There was a container of stainless steel cleaner and a working spray bottle of glass cleaner on the dry storage shelf in the back directly above single service utensils. COS- PIC moved the chemicals to a different area.
- KDA_Defacto - --chicken 175 (grill) --chicken 41 (walk in cooler) sliced tomatoes 41 (make table front) chicken salad 40 (make table 2 front) lasagna 41 (reach in drawer 1) raw salmon 36 (reach in drawer 2) beef 39 (reach in drawer 3) --soup 161 (steam table 1 front) lobster bisque 168 (steam table 2 front) --The hot water in the dishmachine measured at 172.2F. The QT measured at 200ppm in the 3 vat sink at the bar, no test strips provided. --There is a pest control company contracted, no paperwork provided. No activity. --There were containers of Raid flying insect spray on the shelf in the back that were listed as commercial products (EPA Reg 4822-569).
