- KDA_2_30114F - --Employee used gloves to crack a raw egg on the flat top grill and then grabbed a plate to put pancakes and bacon on it without removing his gloves and washing his hands. He then went and put a dollop of butter on the plate and placed the plate in the window for server to take out to customer. Corrected on site (COS)- educated employee on when to wash hands
- KDA_3_30211A1b - --Located in refrigerator 1 in the storage room, there were raw shell eggs stored over a drawer of packages of ready to eat butter. COS- moved butter. Observed by KDA 13. In the True reach in cooler (RIC), there was a bowl of French toast sauce containing raw eggs that was being stored on the top shelf of the cooler above packages of cooked sausage, raw vegetables, and other ready to eat foods. COS- moved French toast sauce to bottom shelf. Observed by KDA 13.
- KDA_3_30511A1 - --There were three case of soda bib syrup stored on the floor in the storage room.
- KDA_4_20211A2 - --Located on the storage rack above the three compartment sink, there were three plastic containers that had cracks or broken corners on them. COS- discarded.
- KDA_4_50111A - --The plastic insert on the top door of the chest freezer in the storage room was held together with the freezer door with duct tape.
- KDA_4_50111B - --The seal on the freezer door in the kitchen was broken. The seals on refrigerator 1 in the storage room were broken.
- KDA_4_60111A - --On the dish storage rack located next to the stove, there were approximately 15 plates, 8 metal containers, and 2 bowls stored as clean that were soiled with food debris. COS- moved to wash area. Located on the floor next to the three compartment sink, there were several buckets that the PIC stated were used for pancake batter stored as clean that were soiled with dried food debris. COS- moved to wash area. Located on the waitress counter under the pass through window from the kitchen, there were 7 plates stored as clean that were soiled with dried food debris. COS- moved to wash area.
- KDA_4_90311A - --Located in the storage room, there was a box of Styrofoam cups and a sleeve of Styrofoam cups that were being stored on the floor.
- KDA_4_90311B - --Located above the three compartment sink, there were several plastic containers stored as clean that were stacked wet. COS- moved to wash area.
- KDA_4_90311C - --Located on a shelf under the waitress station counter, there were stacks of single use Styrofoam to go containers that were being stored not inverted. COS- inverted.
- KDA_5_20515B - --There is a leak in the faucet of the mop sink.
- KDA_6_20215A3 - --The back door and screen door located in the kitchen area had half inch air gaps on the side where the doors meets the door frame.
- KDA_6_30114 - --There was no hand sink posted at the kitchen hand washing sink. COS- provided person in charge (PIC) with sign. Observed by KDA 13.
- KDA_6_50111 - --There is a hole in the sheetrock behind refrigerator 2 and a hole in the ceiling above refrigerator 2 in the storage room. There is a hole in the wall in the mop sink room. There is paint peeling on the kitchen floor. There is a hole in the wall next to where the bread rack is stored in the kitchen. There is paint peeling on the ceiling in the dining room. There are water stains on the ceiling in the room with the HVAC system.
- KDA_6_501111A - --In the mechanical room with electrical box located in the kitchen, there were approximately 20-30 fresh rodent droppings on the floor.
- KDA_6_50112A - --There is an accumulation dried food debris along the walls of the kitchen. There is an accumulation of food debris in the corner behind the stove.
- KDA_7_20111B - --In the storage room, there were bags of liquid hand soap stored on a shelf above shelves with single use plates and bowls. COS- moved hand soap. In the storage room, there were boxes of liquid fuel stored above the shelf of canned food items. COS- moved liquid fuel. Observed by KDA 13.
- KDA_7_20911 - --Located on the waitress counter, there were two bottles of lotion and a small hand mirror stored on the counter used for food preparation. COS- moved personal items to bottom shelf under counter.
