  • KDA_3_30212 - --In the server station, there were three clear bottles with brown liquid in them with out a common name. Employee said they were syrup. Also a container with a brownish/gray powder with a common name. Employee said it was a diabetic customers hot chocolate. COS-containers were labeled.
  • KDA_3_50116A2 - --In the make table, shredded lettuce 53F, sliced tomatoes 47.7F and shredded cheese 53.4F. Employee said this items had been out since 10:00AM. The make table is not working, they have these items on ice. COS- They put the food containers in a larger pan and added more ice so it go higher on the containers.
  • KDA_3_50118A2 - --In the dining room True reach in cooler, the following items were with out a date mark: hard boiled eggs, uncooked peas, cottage cheese, shredded lettuce, cooked on site breaded and shredded chicken. Person in charge said, they fill up the salad bar crocks on Saturday and keep for four days and then discard. COS-the following items were discarded, peas, hard boiled eggs and shredded lettuce. The other items were dated with correct date. Person in charge is going to make a notebook with current salad bar items and there prep and discard dates. Since Person in charge knew when these items were prepared and they were not held over seven days, inspector issued a field warning letter.
  • KDA_4_20211A2 - --Above the three compartment sink, there were three plastic containers that had cracks across the food contact surface. On a shelf above the chest freezer in the kitchen, there was one plastic container that had cracks across the food contact surface. Stored in a container at the end of the three compartment sink, a rubber spatula had cracks across the food contact surface. COS- these items were removed from use.
  • KDA_4_30111 - --The make table that is used to hold the condiments, is not working. They put the potentially hazard foods on ice.
  • KDA_4_50111A - --The rubber seal on the left door of the True reach in cooler in the dining room is loose.
  • KDA_4_60111A - --Above the three compartment sink, there was one brown crock that had sticker residue on the outside. COS-crock was placed at three compartment sink.
  • KDA_4_60111C - --At the end of the three compartment sink, a white oval container that is used to drain the utensil after they are washed had an accumulation of dirt and debris. COS-container was washed. The fronts and the handles on the dining room True reach in cooler and the make table front and handles have an accumulation of grease and debris. The oil pan that sits on the stove top, has an accumulation of dried oil on the outside. The toaster has an accumulation of dried on grease and food debris on the top and sides. The stove, the grills have an accumulation of dirt and grease debris on the tops, sides and fronts. The flat top grill also has accumulation of grease and dried on food on the back ledge. The fryer fronts have an accumulation of grease and dried on food debris.
  • KDA_4_60212B - --The interior walls and door of the microwave has dried on food debris. Person in charge said it was last cleaned on Saturday.
  • KDA_6_20214 - --The men's restroom door is not self closing.
  • KDA_6_50111 - --In the women's restroom, there are four ceiling tiles with brown stains on them. In the men's restroom, there are two ceiling tiles with brown stains. There are two loose ceiling tile in the hallway by the restrooms. There is cracked floor tile, in the server station, in front of the entry to the kitchen and around the front door. The kitchen floor is missing floor tile through the kitchen, leaving exposed concrete. There is one ceiling vent by the front door that is being held in place by a strip of wire.
  • KDA_6_50112A - --The hallway carpet has an accumulation of dirt. The exterior of the wooden slat doors, have an accumulation of dried on food. There is an accumulation of dirt and debris along the wall edges of the kitchen and storage rooms. In the dining area, the ceiling vents have rust and dust. The following walls have dried on food debris, to the east of the grill, above the chest freezer in the kitchen and above the make table. The grill shelf has an accumulation of dirt, grease and dried on food debris. There is an accumulation of dirt, grease and food under the fryers, grills, three compartment sink and beside the True reach in, in the dining area. There is accumulation of dirt behind the chest freezer in dinning room area. There are seven old rodent dropping in the left corner of the small storage room.

  • KDA_6_50119 - --The men's restroom door was open during business hours. COS-door was closed.
  • KDA_7_20411 - --In the sanitizing cloth bucket, the chlorine level was over 300PPM. Employee said she uses a capful of bleach to her container. COS-chlorine level was corrected to 100PPM.
  • KDA_Defacto - --Frozen food is frozen. --In the steam table, chicken fried steak 157.2F. --Kitchen True reach in, 1/2 commercially prepared ham 39.9F. In the dining area True reach in, shredded lettuce 39.4F. --On the grill, hamburger patty 181.5F.
