COS- labeled. KDA_3_30511A1 - --Located in the walk in freezer (WIF), there was a box of burritos stored on the floor. KDA_4_20211A2 - --Located in a drawer under the kitchen prep counter at the back of the kitchen, there were two rubber spatulas that were chipped. COS- discarded. KDA_4_60111A - --Located in a drawer under the prep counter at the back of the kitchen, there was a pair of tongs stored as clean that were soiled with dried food debris. COS- moved to wash area. KDA_4_60111C - --The fan guards of the WIC are soiled with an accumulation of dust. KDA_4_60211E4 - --There is mold in the soda nozzles at the customer self serve soda fountain in the retail area of the establishment. There is mold on the inside rim of the ice machine located in the storage room. KDA_4_90311A - --Located in the outside storage shed, there were two boxes of single use cups and two sleeves of pizza boxes stored on the floor. KDA_5_501113 - --The lid to the outside dumpster was open. KDA_7_20111B - --Located in the storage room, there was a container of stainless steel maintenance spray and a bottle of weed killer stored on a shelf above a shelf of single use cups. COS- moved. KDA_Defacto - --ambient air temperature of WIC- 41.9F --bacon pizza 198F --Hatco warmer- breakfast pizza 148F --MT- sausage crumbles 25F, beef crumbles 30F, pizza sauce 36F, RIC under MT- ham slices 40F, roast beef slices 41F, retail sandwich RIC- tuna salad sandwich 42F, WIC- tuna salad 43F, shredded cheese 43F, coffee creamer/half and half machine- half and half 42F, --three vat sink QT sanitizer concentration- 200ppm (per label) KDA_KSA65_657b - --Located in the retail area, there was one box of Alka Seltzer tablets with an expiration date of 1/13.
