  • KDA_3_10111 - --Located in the bar storage closet, there was 1/4 a bottle of Jack Daniels Whiskey, 1/2 bottle of Jim Beam Bourbon, 2/3 bottle of Bacardi Rum, 1/6 bottle of Crown Royal Whiskey, and a full bottle of Dekuyper Crème of Banana with winged insects inside. Corrected on site (COS)- put in office for personal use.
  • KDA_3_30212 - --Located above the kitchen MT, there was an unlabeled bottle of brown liquid and two unlabeled bottles of yellow liquid which the kitchen employee identified as balsamic vinegar, olive oil, and olive oil with spices, respectively. Located on the baking shelf, there was an unlabeled shaker container of brown powder which the kitchen employee identified as cinnamon sugar. Located in a cabinet at the front counter, there was an unlabeled bottle of white powder and an unlabeled container of black powder which the person in charge (PIC) identified as salt and pepper, respectively.
  • KDA_3_30412C - --There was an ice scoop stored directly on the ice machine.
  • KDA_3_50116A2 - --Located on the counter next to the kitchen make table (MT), there was a container of house made Caesar dressing at 49F. Kitchen employee indicated that it had been out for 10 minutes. COS- put in reach in cooler (RIC) under MT. Located in the MT, there was a container with pre-portioned roast beef slices at 51F. The container was inside another container that had cheese slices in it. Kitchen employee indicated that it had been moved from it's original spot in the MT about 20 minutes ago. COS- moved back to original spot in the MT in the container that is further down in the MT.
  • KDA_3_50118A1 - --Located in the True RIC in the kitchen, there was a container of queso sauce that was dated from 10/1 through 10/7. Kitchen employee verified date. COS- discarded. (1 out of approximately 100 date marked items)
  • KDA_4_20211A2 - --Located on the dish shelf at the back of the kitchen, there was one wooden spoon that was chipped and one rubber spatula that was pitted. COS- discarded.
  • KDA_4_50111B - --The seal to the True RIC is broken. The seal to the Arctic Air reach in freezer (RIF) is broken. The seals to both of the True RIFs are broken. The seal to the True RIC at the bar area is broken.
  • KDA_4_60111A - --Located on the dish rack at the back of the kitchen, there was one ladle, one pair of tongs, and two knives stored as clean that were soiled with dried food debris. COS- moved to wash area. Located on the dish shelf at the back of the kitchen, there were three plastic food container lids stored as clean that were soiled with sticker residue. COS- moved to wash area. The sandwich press in the kitchen was soiled with dried cheese. COS- moved to wash area.
  • KDA_4_60111C - --The vents at the back of the counter RIC are covered in dust. There is dried food debris accumulating at the bottom of the kitchen oven.
  • KDA_4_60211E4 - --Located in the bar area of the kitchen, there was mold on the inside rim of the ice machine. There is mold on the nozzle of the bar area soda fountain. There is mold on the soda nozzles of the front counter soda fountain.
  • KDA_6_50112A - --There is debris on the floor behind the hot water heater in the storage room.
